Kenntnisprüfung or Anpassungslehrgang? Choosing the Compensation Measure for Indian Nurses

24 October 2026  ·  Taha Airwaves

Most Indian nurses who apply for German recognition do not receive full equivalence straight away. They receive a notice identifying differences from the German Pflegefachfrau or Pflegefachmann training and are offered a compensation measure. There are two: the knowledge test (Kenntnisprüfung) and the adaptation course (Anpassungslehrgang). Hospitals and care providers often leave the choice to the nurse, but the employer's circumstances matter just as much.

The Knowledge Test

The Kenntnisprüfung is an examination built around the content of the German state nursing exam. It normally has an oral part and a practical part on a ward, assessed by an examination board. Preparation courses run from a few weeks to several months, typically combining classroom teaching on German law, documentation, care planning and expert standards with supervised ward practice.

Strengths: it can be quick for experienced nurses with strong clinical skills and good German; it does not depend on the size of the gaps identified.

Weaknesses: it is a single high-stakes exam; nervous candidates or those whose German is still developing can struggle, and failed attempts cost months.

The Adaptation Course

The Anpassungslehrgang is a period of supervised practice and, where needed, theory, focused on the gaps listed in the deficit notice. It ends with a final assessment, usually a discussion with the examination board rather than a full exam. Its length depends on the gaps; six to twelve months is common, and the legal maximum is longer.

Strengths: learning happens on the job; candidates are assessed on what they have done over months rather than a single performance.

Weaknesses: it needs placements across the specific areas listed in the notice — paediatric or psychiatric care, for example — which a single care home may not offer. It ties up practice instructors for longer.

A Recent Change Worth Knowing

Amendments to the nursing law in 2023 allow applicants to waive the detailed individual equivalence review and proceed directly to a compensation measure. For many Indian applicants whose files would end in a compensation measure anyway, this can save months. How each state authority implements the option varies, so check the current practice with the competent authority before advising candidates.

What the Employer Should Weigh

Ward and placement capacity. An adaptation course needs rotations that match the deficit notice. Hospital groups with several departments can usually provide them; standalone care homes often cannot without partner institutions.

Practice instructors. Both routes need qualified Praxisanleiter. Count how many hours they can realistically give.

The nurse's German. Strong B2 or better favours the knowledge test. Nurses still building confidence often do better in the course.

Cohort size. With a group of ten or more, a structured preparation course for the knowledge test — run in-house or with a provider — tends to be efficient. For one or two nurses, the course route may be simpler to organise.

Time pressure. If ward staffing depends on fully licensed nurses by a specific date, the knowledge test is usually faster when it works — but plan for a resit.

How Pay Works in Between

Until full recognition, nurses are employed in an assistant or pre-registration grade. Contracts should state the pay before and after recognition, and the expected date. Ambiguity here is a common source of frustration. Our nurse recognition guide explains the stages from the nurse's side.

A Simple Decision Rule

  • Experienced nurse, strong German, employer without full rotation capacity: knowledge test with a good preparation course
  • Less experienced nurse, or German still developing, employer with multiple departments: adaptation course
  • Large cohort: knowledge test preparation as a group, with the adaptation course as a fallback for those who need it

Preparation Matters More Than the Choice

Whichever route is chosen, outcomes depend on preparation: German nursing terminology, documentation practice, knowledge of German expert standards, and confidence speaking to patients and colleagues. Starting this in India, during language training, shortens everything after arrival.

Our Role

We prepare recognition files, brief candidates on both options before departure, and coordinate with employers on placement planning. Combined with the recognition partnership, this lets nurses work in an assistant role while completing the measure. See our nurse recruitment page.

Frequently Asked Questions

Which is faster, the Kenntnisprüfung or the Anpassungslehrgang?

The knowledge test is often faster for experienced nurses with strong German, because it does not depend on completing placements. The adaptation course usually takes six to twelve months but carries less single-exam risk.

Can a nurse work while completing the compensation measure?

Yes. Nurses usually work in an assistant or pre-registration grade, often under a recognition partnership, until the licence is issued.
