Health Insurance and Social Security in Germany: What Indian Workers Pay and What They Get

Indian workers comparing German and Gulf payslips notice one thing immediately: the deductions. Germany takes roughly 20 percent of gross salary in social contributions before income tax, and the employer pays a similar amount on top. In return you receive a level of protection no Gulf contract offers. This guide explains each line.

Statutory Health Insurance (Gesetzliche Krankenversicherung)

Mandatory for employees earning below the insurance threshold, which covers virtually every trade worker. The contribution is about 14.6 percent of gross plus a fund-specific supplement, split equally between you and the employer. It covers doctor visits, hospital treatment, most medicines, dental basics, maternity and sick pay after the employer's six weeks of continued salary. Your spouse and children, once in Germany, are covered free of charge under family insurance if they have no income of their own.

Pension Insurance (Rentenversicherung)

18.6 percent of gross, split equally. Every month of contribution earns pension entitlements that follow you for life.

The India–Germany Social Security Agreement

India and Germany have a bilateral social security agreement in force. For workers on regular German employment contracts, it ensures contribution periods in both countries can be combined to meet eligibility rules, and pensions are paid to India. Workers who leave Germany after fewer than five years of contributions and return to India can apply for a refund of their own pension contributions after a waiting period of 24 months — a substantial sum that many Indian workers do not know they are owed.

Unemployment Insurance

2.6 percent, split equally. After twelve months of contributions you are entitled to unemployment benefit if you lose your job through no fault of your own — a safety net that also gives time to find a new employer under FEG rules.

Long-Term Care Insurance (Pflegeversicherung)

Around 3.6 percent, with a small surcharge for those without children. It funds care in old age or disability.

Accident Insurance

Paid entirely by the employer. Covers workplace and commuting accidents, including rehabilitation and pensions for permanent injury — significant for construction and industrial workers.

Income Tax

Applied after social contributions, on a progressive scale with a tax-free allowance. Single workers are in tax class I; married workers whose spouse joins them can switch to a class that reduces the monthly deduction.

Private Versus Statutory Insurance

Workers earning above the statutory threshold (well over €70,000 in 2026) may choose private insurance; almost no trade or care worker reaches it, and for everyone else statutory insurance is compulsory and better value once family joins. Ignore agents in India selling "German private health plans" for work visas — the visa section requires proof of statutory or equivalent cover for the first weeks only, and your employer's enrolment replaces it.

Sick Pay and Leave

If you fall ill, the employer continues full salary for six weeks; the health fund then pays roughly 70 percent of gross for up to 78 weeks. Statutory paid holiday is at least 20 days on a five-day week and usually 25 to 30 under collective agreements. Maternity and parental leave are paid entitlements, not favours.

What to Check on Your First Payslip

Gross salary as per contract, health fund name, pension contribution line and the net figure. Errors are rare but easy to fix in the first month and painful later.
