Medical Exam, Fingerprints & VHI for Workers in Russia

24 October 2026  ·  Taha Airwaves

When Indian workers land in Russia, they cannot simply walk onto the job. Before the work permit is issued, each worker must complete a medical examination, fingerprinting and photographing, and have valid voluntary medical insurance. These steps are routine, but they have deadlines and they take time. Employers who plan them like a project get workers productive weeks earlier.

Step One: The Migration Medical

Foreign nationals coming to work must undergo a medical examination at an authorised medical organisation. The examination typically checks for infectious diseases such as tuberculosis and HIV, and for drug use. Rules on where and when it must be done have been tightened, and there are deadlines from the date of entry, so the medical must be scheduled promptly after arrival.

Practical points:

  • Book appointments for the whole batch before workers land
  • Arrange transport and a coordinator to accompany the group
  • Bring passports, migration cards and any required translations
  • Keep copies of results for the employer's files

A pre-departure medical in India — which we arrange as standard — does not replace the Russian examination, but it does screen out candidates likely to fail, saving everyone time and cost.

Step Two: Fingerprinting and Photographs

Foreign workers must also undergo fingerprint registration and photographing as part of the permit process. This is done through the migration authorities and is usually combined with the permit application. It is quick, but queues can be long in large cities.

Step Three: Voluntary Medical Insurance (VHI)

A foreign worker on a temporary stay in Russia must hold a voluntary medical insurance policy (in Russian, DMS) or be covered by an agreement between the employer and a medical provider. VHI is required for the work permit and must stay valid for the whole employment. Employers usually buy a group policy through one insurer.

Check that the policy covers primary and emergency care, that workers know how to use it, and that it is renewed before expiry. Lapsed insurance can undermine the worker's legal status.

Step Four: The Work Permit

Once the medical results, fingerprinting and insurance are in place, the work permit is issued. Only then can the employment contract be performed in full, and the employer must send the 3-working-day contract notification. See our registration and notification checklist.

Food and Healthcare Workers

Workers in food production, catering and some other roles also need a personal medical record book under sanitary rules. This is a separate requirement from the migration medical and has its own tests. Book it in the same first weeks.

A Typical First Fortnight

  • Day 1: Arrival, accommodation, migration registration started
  • Days 2–5: Migration medical for the batch
  • Days 3–7: Fingerprinting and permit documents submitted
  • Days 7–14: Work permit issued; contract notification filed; safety induction
  • Ongoing: Medical record book for food workers; job training

Actual timing depends on the region and appointment availability.

What Can Go Wrong

  • A positive medical result. The worker may not receive a permit. Pre-departure screening in India reduces but cannot eliminate this risk. The service agreement should say what happens next, including a replacement.
  • Missed deadlines. If the medical or documents are not submitted within the required period, the worker's position becomes complicated. Plan the first days carefully.
  • Lapsed VHI. Track renewal dates on the same list as permits and visas.

How We Prepare Workers in India

Before departure we explain each step in Hindi or English: why the Russian medical exists, what fingerprinting involves, what VHI is and how to use it. Workers who understand the process are calmer and cooperate better. We also supply each employer with a batch list containing passport details, arrival dates and pre-departure medical summaries.
